Foreword 



This publication is one of a monthly series containing data on 
medical assistance payments made under Title XIX of the Social 
Security Act (Medicaid). Information is compiled and reported 
by each State and/or jurisdictional agency administering an 
approved Title XIX program on Federal Report Form SRS-NCSS-120: 
Monthly Statistical Report on Medical Care. Currently there 
are 54 such programs in 49 States, the District of Columbia, 
Guam, Puerto Rico and the Virgin Islands. Arizona has not 
initiated a Title XIX program and Massachusetts has a separate 
State agency for blind recipients. 

Prior to January 1974, similar data were collected for the mid- 
month of each quarter and midmonth quarterly reports have been 
published in this series since May 1967. 

Reports for the months of March, June and September will include 
quarterly summary data and in addition, reports for December 
will include annual summary data. 

Information presented include medical payments by form of 
payment, payments to medical vendors by type of service, number 
of recipients in whose behalf medical vendor payments were 
made, and data related to screening services under the Early 
and Periodic Screening, Diagnosis and Treatment Program.

IMttODUCTION 



Medicaid (Title XIX of the Social Security Act) is an assistance 
program designed to provide medical assistance to specified groups 
of needy individuals. Such persons are classified into two main 
groups according to their maintenance assistance status, (1) those 
who are authorized to receive money payments for their basic 
necessities under public assistance programs such as Supplemental 
Security Income and Aid to Families with Dependent Children, and 
(2) those who are not receiving maintenance assistance payments even 
though they may, in fact, be eligible. There are extensions of the 
maintenance assistance status categories but uniform procedures do 
not exist for reporting such data. There are four basic eligibility 
requirements for medical assistance: (1) age 65 and over; (2) blind- 
ness; (3) permanent and total disability; and (4) membership in 
families with dependent children. In addition, some States have 
exercised the option of including other needy persons who do not fall 
into any of the above categories. Such recipients are classified as 
'Other title XIX' recipients in the accompanying tables. Also, 
'essential persons' (usually spouses) may also be eligible for medical 
care. For money payment recipients, these persons are usually 
classified according to the same eligibility factors as the recipient 
spouse. 

Generally, payments are made out of Medicaid funds to vendors 
(providers) of medical services, directly or through a fiscal agent as 
the direct results of claims filed by the provider after services are 
rendered. Data presented in this publication are generated as an 
adjunct to the Medicaid claims processing and payment operation. Thus, 
information is based on bills paid and not services rendered during the 
month. Consequently, the data provided through this process are not 
professed to represent a true picture of the Incidence of illness among 
Medicaid eligibles and are subject to fluctuations unrelated to the 
provision of medical care. Monthly data are collected and presented to 
meet pressing current demands for minimum information on medical care 
financed under title XIX. However, States are required to provide more 
significant and detailed annual data on the numbers of persons receiving 
medical services, the number of units of such services and the corre- 
sponding amounts of payments. 

Except for payments, data related to screening services are not 
necessarily generated through the claims payment process. Numbers of 
individuals screened are usually based on services actually rendered 
during the report month. As a result, payments reported for screening 
services may be unrelated to persons receiving such services. 



HIGHLIGHTS 



Medical Assistance (Medicaid) Financed Under Title XIX 
of the Social Security Act - September 1977 



Payments made out of Title XIX funds in September totalled $1,443 
billion, a decrease of $39.5 million from August. Average monthly 
payments for the twelve month period ending in September equalled 
$1,398 billion, an increase of 0.6 percent over the twelve month 
period ending in August. 

Approximately 8.7 million recipients 1/ had vendor payments total- 
ling $1,410 billion made in their behalf. 

There were concurrently 77,650 Medicaid eligibles enrolled in health 
maintenance organizations whose premiums totalling $2.7 million 
were paid out of Title XIX funds. 

Persons Served 

The percentage distribution of persons for whom medical payments 
were made by basis of eligibility and money payment status for 
September is as follows:

More than half (54.3 percent) of the recipients for whom vendor 
payments were made were members of families with dependent children. 
Children under 21 in families and other Title XIX recipients under 
21 accounted for 36.0 percent of recipients. Recipients eligible 
on the basis of age 65 and over accounted for 22.7 percent of the 
case load. Recipients who also received money payments outnumbered 
nonmoney payment recipients by more than 3 to 1. Among members of 
families with dependent children under 21, nine out of ten Medicaid 
recipients also received AFDC money payments. 

Medical Payments by Groups of Persons Served 

As shown below, the distribution of payments differs significantly 
from the case load distribution. Payments for persons not receiving 
maintenance assistance are nearly equal to those for money payment 
recipients. The single largest proportion of payments (26.1 percent) 
are made on behalf of recipients aged 65 and older who are not also 
recipients of maintenance assistance. Such recipients represent only 
8.5 percent of the caseload. Payments for these recipients, to a 
large extent, are made to medical inpatient facilities which re- 
present the more expensive types of medical services. Only 17.4 
percent (about half of the corresponding recipient proportion) of 
payments are made on behalf of children under 21 years of age. The 
second largest proportion of payments (27.8 percent) are made on be- 
half of the permanently and totally disabled.

Payments by Type of Service 

Inpatient services comprised 71.5 percent of medical vendor 
payments in September: hospitals (31.0 percent), skilled nursing 
facilities (16.8 percent), intermediate care facilities (23.7 per- 
cent). Physicians' services, prescribed drugs and outpatient 
hospital services accounted for 8.9,6.1, and 5.1 percent res- 
pectively, of the total payments. 

Screening Services 

During September 161,658 children were reported as screened under 
the Early and Periodic Screening, Diagnosis and Treatment Program. 
About fifty-one percent of the children screened were six years of 
age or older. Forty-nine percent of the children screened had re- 
ferrable conditions uncovered and ninety-one percent with such 
conditions uncovered were referred for diagnosis and treatment. 



Medical Assistance Financed Through State

During the month of September 1977, twenty-one States reported 
a total of $71,815,284 in payments for medical assistance which 
were not subject to Federal financial participation.

1/ States are asked to report medical payments made from general 
assistance funds separately from Title XIX. Thus, the data 
are excluded from the detailed tabulations. However, New York 
reports combined data and Hawaii, Maryland and Utah included 
the information in the Title XIX statistics prior to July 1977. 



ANNOTATIONS 



1. The following definitions apply to symbols appearing in 
the detailed tables: 

indicates no data reported. 

Z indicates that the percent was less than 0.05. 

2. Arizona has no Title XIX program and is excluded from 
the tabulations. 

3 , Oklahoma discontinued its agency pooled fund beginning 
July 1976. 

4. All data for Tennessee were estimated by the State. 

5. Georgia did not report recipients or payments by maintenance 
assistance status and basis of eligibility of recipient 

and is excluded from all applicable tables. Wisconsin re- 
ported payments by maintenance assistance status and basis 
of eligibility of recipient only. Colorado reported re- 
cipients and payments by basis of eligibility only. 

6 States do not always report premium or per capita payments 
into the SSA system for aged and disabled recipients 
separately. Payments reported under the aged category in- 
clude payments for the disabled. There is variance from 
month to month in States reporting SSA and HMO premium 
payments because in some cases, such payments may be made 
less frequently than monthly (Table 1) . 

7. Pennsylvania recipients for prescribed drugs are not in- 
cluded in the tabulations. New Hampshire payments for 
medical transportation and Colorado payments for dental 
service are reflected in Tables 1 and 5 only. 

8. For Massachusetts, payments for intermediate care facility 
services for the mentally retarded represent July through 
September total. Such data are available at the end of 
the quarter. 

9. Data for Idaho in Tables 2-14 include children screened 
under the EPSDT program and payments for such services.

10. In addition to Colorado, Georgia, and Wisconsin; Massachusetts, 
Nevada and Pennsylvania are excluded from Tables 10-13 because 
children and adults in families were not reported separately. 
Connecticut, Illinois, and West Virginia were able to report 
children and adults separately but were unable to differentiate 
medical vendor payments between adults and children in families. 
These States are excluded from the total percentage computation 
in Table 11. Colorado and Pennsylvania did not differentiate 
other Title XIX recipients by age and are excluded from Table 14. 

11. In some States, two or more types of medical services may be 
grouped together under one class, such as Connecticut where 
other practitioners 1 , clinic, laboratory and radiological, home 
health, and family planning services are included in other care, 
and South Carolina, where skilled nursing facility services in- 
clude payments to State mental institutions. 

12. For some States, difference in vendor payment amounts between 
Table 3 and Table 5 are due to inclusion of retroactive ad- 
justments which can be reported by type of service (Table 5) 
but which are not applicable to the recipients reported in 
Tables 2, 4, and 6-9. For the current month, however, ex- 
penditures for Michigan exclude accounting adjustments of 

(-$861,888), 

ANNOTATIONS SPECIFIC FOR SCREENING TABLES 15-20 

13. States omitted from tables either did not report on screening 
services for September 1977 or there were no individuals screened 
during the month. 

14. California reported 96,261 individuals receiving comprehensive 
physical examinations in addition to the screening data. 

15. Maryland reported 234 individuals screened in HMO's, but such 
cases were not distributed by age. 

16. Included in the totals for Minnesota are 127 individuals screened 
at C and Y projects and for Rhode Island are 456 individuals in- 
cluding 137 newborns for whom there is no detailed information, 

17. Indiana reported incompletely with respect to referral status of 
children screened. 

18. Florida, Hawaii, Idaho, Maine, Massachusetts, North Carolina, Ohio, 
Pennsylvania, Vermont, Virginia, Washington and Puerto Rico re- 
ported incompletely with respect to problems uncovered during 
screening. 

19. Texas data do not include 12,194 dental screenings which are per- 
formed separately. 

20. Payments for screening services were not reported by Massachusetts^! 
and Puerto Rico and payments for Idaho, New York, Michigan, 
Pennsylvania, and Texas were not reported separately by age.

JULY- SEPTEMBER 1977 



Payments made out of Title XIX funds totalled $4.3 billion during 
the last quarter of fiscal year 1977 for a 4.5 percent decrease 
from the previous quarter. Recipients in whose behalf medical 
vendor payments were made averaged 8.8 million persons per month 
which was also a decrease from the prior quarter. The occurrence 
of the decrease in payment amounts and recipients is a usual seasonal 
phenomenon observed for the July through September quarter. However, 
an increase of 8.2 percent in payments occurred over the same 
quarter a year ago. 

Among eligibility categories, members of families with dependent 
children under 21 constituted 54.6 percent of the caseload for an 
average monthly number of 4.8 million persons. Decreases in the 
average monthly number of recipients were ooserved for every 
eligibility category except the blind for whom there was an in- 
crease over the prior quarter. 

The decrease in amounts of payments was larger for non-money payment 
recipients than those who received maintenance assistance. 

Decreased payments from the previous quarter were consistent for. all
